AFib 2gether™ is a shared decision-making mobile platform that may be helpful to both patients and clinicians. AFib 2gether™ helps you build your understanding of stroke risk due to diagnosis of atrial fibrillation, which is a type of irregular heartbeat, not caused by heart valve problem. The goal of the app is to help you have an informed discussion with your health care provider.
For atrial fibrillation patients and caregivers:
You will answer questions which will help you and your clinicians determine your risk of stroke due to atrial fibrillation by providing a personalized risk score calculation. You will also be able to prioritize questions of importance about your atrial fibrillation and stroke risk for discussion with your healthcare provider.
Utilizing the Resources section within the app will help you learn about atrial fibrillation and the risks associated with this condition. You will also have the opportunity to understand the terminology of your condition; view videos that help you understand the condition, and access links to other helpful sites.
For atrial fibrillation providers:
AFib 2gether™ helps support a shared decision making conversation for patients who are diagnosed with atrial
fibrillation not caused by a heart valve problem. In 2016, the ACC/AHA/HRS published quality metrics which highlight shared decision making in atrial fibrillation. The app will assist the patient-provider interaction and help facilitate a discussion about stroke risk due to atrial fibrillation.
